Putting in the CVC requires a minor surgical procedure. Sometimes this is done in a clinic or hospital room and sometimes in an operating room. But once a CVC is in place, you can get all your treatments through the CVC, will not be stuck multiple times for IVs, can have most blood tests drawn through it, and can get other treatments you might need through it, such as fluids, blood transfusions, or antibiotics.

Many people talk about CVC options with their doctor even before starting treatment. Some find out during treatment that they need a CVC because it becomes more difficult over time to find a suitable vein in their hand or arm to use for infusions or injections.

Your health care team can help you decide if you need a CVC and what type is right for you. Intrathecal or IT chemo is put into the spinal canal through a catheter, and goes into the fluid that surrounds the brain and spinal cord, called the cerebrospinal fluid or CSF.

This way of giving chemo may be needed for certain kinds of cancers that affect the brain since most chemo drugs delivered by IV or by mouth cannot pass through the blood-brain barrier that protects the brain from many toxins. When IT chemo is given, it can be delivered to the CSF through a needle placed in the spinal area, or through a long-term catheter and port put under the skin on your head during surgery.

This special kind of port is called an Ommaya reservoir. The Ommaya is a small drum-like device with a small tube attached to it. The tube goes into the CSF in a cavity of your brain. Unless your oncology team says otherwise, you should block out several hours for a chemotherapy infusion. Before the infusion can begin, a few other things must happen. Some appointments will involve meeting with your oncologist. In general, it takes a few minutes for an IV push, while an IV infusion can take 30 minutes to several hours or more.

A continuous infusion can last 1 to 3 days. Chemotherapy usually takes more than one treatment and is typically given in cycles. Your oncologist will base this plan on:.

This rest period is typically 2 to 3 weeks, but it can be shorter or longer. A new cycle begins with your next infusion. Your oncologist will plan for you to have a set number of cycles. For example, your plan might call for 4 infusions with one chemo drug at 2-week intervals, followed by 3 infusions with a different drug combination at 1-week intervals.

Chemotherapy would last about 4 months in this case. A lot depends on how your body responds to the chemotherapy drugs. A blood test before each treatment will show if your white blood cell count or platelet count is low. Your next treatment may be delayed by a week or two. If chemo dose adjustments are needed, that could also affect the number of cycles. Chemo is sometimes used to shrink a tumor before surgery.
